To request a Prescription Plan and/or Advantage Plan Review please follow the below instructions.
First gather the following information:
Prescriptions
You will need a list of prescriptions you take: name, dosage and how per day you take it (or how often you refill if it is as needed prescription).
It is important that you accurately list your prescriptions. Inaccurate prescriptions will result in incorrect review results. Common mistakes include: listing the name brand when you take the generic, choosing a tablet instead of capsule or vice versa.
If you take a prescription “as needed” you will need to make a best guess as to how often you fill it. “As needed” is not an option.
Doctors: Advantage Plan Reviews Only
You only need to list your doctors if you are requesting a Medicare Advantage review.
Prescription plan only reviews do NOT need doctors listed so you can skip this step.
Once you Have Gathered that information:

Click on the link at the bottom of this email and begin to fill out your information. It will ask for your prescriptions, list of doctors and other basic information. The last step of the review link will be the scope of appointment form. Please check all items you think you might want quoted or like to discuss.
If you want to discuss both Medicare Advantage and traditional Medicare be sure to select both prescription drug plan (Part D) and Medicare Advantage Plans (Part C).
