Prescription Plan Review
Step 1: Prescription Plan Review Form and Scope of Appointment Form
Per Medicare guidelines, we are required to have this signed by you prior to discussing your prescription plan or Advantage plan. The Scope of Appointment form is included as the last 2 pages of the forms below. If you are printing and filling the forms out you will need to initial next to Stand-alone Medicare Prescription Plan (Part D); next sign and date under “Beneficiary or Authorized Representative…” .
COMPLETE FORMS BELOW
We prefer you complete these forms using the Complete ONLINE link. It will ask you for your email address then send you a link through your email where you can complete the forms online. If you struggle with this method you can click the Download & Fill Out Forms to print them and fill them out by hand.
ONLY IF YOU DOWNLOADED & COMPLETED FORMS – Step 2: Submit Your Form(s)
If you printed your forms and completed them by hand you can submit them by:

Step 3: We Will Contact You
Once you have submitted your forms we will send you an email confirmation that we have received them within 48 business hours. If you have not received confirmation please resubmit your forms. Please keep an eye on your email for updates and review information if an email address has been provided.
We will work quickly to get everyone’s review done, however, we ask that you please be patient with us. There are ~2,000 of you and 2 of us. We promise to get in touch with you before Thanksgiving, so you may not hear from us for a few weeks. We work through our list of people (and pots of coffee 
) as quickly as we can.
